History/Physical:

34-year-old right-handed female.

Fatigue and visual obscurations over nine months.

On the unenhanced coronal T1, there is a solid/cystic lesion (arrowheads) just inferior to the frontal horns.

Post-contrast, there is enhancement of the solid component (arrow). The fluid component has a thin internal septation. Note the cyst fluid (lower asterisk) is slightly hyperintense compared to the CSF in the frontal horns (upper asterisks). This might be from proteinaceous debris in the cyst fluid.

Diagnosis: Craniopharyngioma

Epithelial nests and ribbons alternating with loose, hypocellular areas.

Wet keratin (*) and squamous epithelium and basaloid layer (arrow).
