History/Physical:

37-year-old right-handed female.

Headaches worsening over the past four months.

There is a small round lesion in the foramen of Monroe, between the frontal horns (*). This is iso-intense to brain.

The lateral ventricles are enlarged (*). The lesion is obstructing CSF flow from the lateral ventricles to the third ventricle.

This lesion is a mass (*). The high signal relative to CSF is from proteinaceous debris.

Small vessels are draped over the surface of the mass (*).
