History/Physical:

10-year-old left-handed female.

Progressive headache, nausea and vomiting over six months.

The pineal region mass (*) is isointense to cortex.

The mass enhances mildly.

The mass may arise from the pineal gland, which cannot be identified separate from the mass.

There is significant mass effect on the superior colliculi (arrow) and aqueduct of Sylvius (arrowhead). There is obstructive hydrocephalus.

The lateral ventricles are enlarged (arrows) and there is transependymal CSF flow (arrowheads).
