History/Physical:

59-year-old right-handed male.

Gradual onset of bi-temporal hemianopsia over two years.

Mild headache.

There is a well-defined mass arising from the pituitary fossa (*). This encroaches on the optic chiasm without impingement (arrows).

The cavernous sinuses are intact (arrows).

The mass enhances homogeneously.

Diagnosis: Pituicytoma

Neurosecretory vesicles (arrow) are present in the cytoplasm of neuronal elements.
