History/Physical:

35-year-old left-handed female.

Gradual onset of right facial numbness.

Diminished hearing in right ear.

Enhancing mass arising from the right IAC extending into the cerebello-pontine angle. There is central irregular low attenuation compatible with tumour necrosis (*).

There is a CSF cleft (arrowheads), indicating this mass is not arising from the pons or cerebellum. Note the normal left CN VII/VIII complex (arrows).

Diagnosis: Schwannoma

Biphasic architecture of compact (Antoni A) and loose areas (Antoni B).

Prominent nuclear pallisades (arrow).
