History/Physical:

69-year-old right-handed female.

Rapid onset of bi-temporal hemianopsia.

Mass in the pituitary fossa causes bony remodelling (arrows).

The mass (*) arises from the pituitary fossa and extends into the suprasellar cistern.

This view shows the 'snowman' sign. The mass has a waist (arrows) from the diaphragma sella, which is an extension of relatively rigid dura. There is mass effect on the optic chiasm (arrowheads).

Diagnosis: Pituitary adenoma, null cell
