History/Physical:

59-year-old right-handed male.

Mild gait ataxia over two months.

axial T1 MRI + contrast

Small ring-enhancing mass in the posterior right cerebellum. The rim is smooth. There is no significant mass effect.

Tx: Suboccipital craniectomy and removal of lesion.

Diagnosis: Metastatic adenocarcinoma

Tumour deposit is sharply demarcated from cerebellar cortex (top).

Tumour cells in crude lobules and acini display cytoplasmic clearing and prominent nucleoli.

Nuclei express thyroid transcription factor-1 (TTF-1).

Lesional cells express cytokeratin-7 (CK7) but not cytokeratin-20 (not shown).
(Immunomorphology consistent with a pulmonary primary)
