History/Physical:

51-year-old left-handed male.

Optometrist noted bitemporal hemianopsia on routine examination.

The mass in the pituitary fossa on the left side (*) invades the cavernous sinus and surrounds the left internal carotid artery (arrow). There is mass effect on the left side of the optic chiasm (arrowhead).

The enhancing mass (*) mildly expands the sella turcica.

The mass (*) enhances less than normal pituitary tissue (arrowhead).

Diagnosis: Pituitary adenoma, gonadotroph lineage
