History/Physical:

51-year-old right-handed male.

Progressive speech difficulties over the past two months.

The cystic lesion (*) in the left temporal pole has only minor surrounding edema and mass effect.

There is only thin smooth rim enhancement.

Mild effacement of the Sylvian fissure is present (arrowhead).

The cyst contents have increased signal (*) compared with simple CSF (arrow).

Diagnosis: Metastatic small cell carcinoma (lung primary)
