History/Physical:

70-year-old right-handed male.

Recent onset of thoracic back pain.

Lateral foot x-ray

T1-hypointense lesions are easily identified on the background of bright fatty marrow.

The lesions are hyperintense on the T2 sequence with fat suppression. There is no significant spinal canal stenosis or cord compression.

Lateral foot radiograph shows an ovoid soft tissue mass arising from the plantar surface.

Tx: Thoracic laminectomy and biopsy of spinal lesion.

Diagnosis: Metastatic sarcoma, undifferentiated/unclassified (foot primary)

Moderately pleomorphic tumour cells with spindle to epithelioid appearances.

Higher magnification of the same.

Vimentin expression among tumour cells is abundant.
