History/Physical:

51-year-old right-handed male.

Bilateral leg weakness increasing over two months.

Increased reflexes in lower extremities.

Previous thoracic laminectomies.

There has been previous laminectomy for tumour resection (arrows). There is a recurrent mass infiltrating the mid-thoracic vertebral body (*).

There is a recurrence in the spinal canal/foramen which enhances (arrow).

Compared with T1, the bony recurrence is less obvious than on T2 (*). However, the recurrence in the spinal canal/foramen is more conspicuous (arrow).

There is irregular enhancement of the tumour recurrence in the vertebral body (*)and foramen (arrow). There is paraspinal component with solid enhancement (arrowhead).

Diagnosis: Chondrosarcoma

Irregular islands of chondroid tissue (*) are separated by fibrovascular bands and populated by clustered cells resembling chondrocytes.
