History/Physical:

66-year-old left-handed male.

New onset generalized tonic-clonic seizure.

There is a complex cystic lesion (*) with mildly hyperintense rim in the left frontal lobe.

Most of the rim is fairly thick and smooth (arrows). The rim is more complex superiorly.

There is prominent vasogenic edema (arrows) causing mass effect.

Diagnosis: Inflammatory tissue with features of early abscess wall formation

The area bordering the necrotic core is dominated by polymorphs (arrows) and spindle cells (likely fibroblasts).
