History/Physical:

7-year-old right-handed female.

Low back pain and mild bilateral leg weakness for two months.

Subtle hypo-intense ovoid mass in the spinal canal (*).

The mass (*) is mostly high signal on T2 with some low signal septations.

The hypointense mass is within the thecal sac (*). Epidural fat surrounding the thecal sac has high signal (arrows).

Diagnosis: Epidermoid

The cyst wall is composed of a mature keratinizing squamous epithelium (*).
