History/Physical:

60-year-old left-handed male.

Right hemianopsia progressive over three months.

Fullness in the left parietal-occipital lobe is evident (*).

Post-contrast, abnormalities in both caudate heads (arrows) and the right thalamus (*) are more apparent.

The left parietal-occipital lesion enhances superiorly (*). There is a small enhancing lesion in the right parietal lobe.

Diagnosis: Primary CNS lymphoma, diffuse large B cell

Perivascular tendency for tumour cell aggregation with intervening areas of necrosis (*).
