History/Physical:

39-year-old right-handed female.

Progressive leg weakness over the past two years.

Mildly increased reflexes in both lower extremities. There was a mid-thoracic sensory level.

There is an enhancing mass in the spinal canal at T5 (*).

The mass extends into the right neural foramen at T5/6 (*). The cord is compressed and displaced to the left (arrow).

Diagnosis: Meningioma

This lobular lesion with whorls and psammoma bodies has generous extracellular collagen (*).

Monotonous histology with psammoma bodies (*), subtle whorls (arrowhead) and intranuclear pseudoinclusions (arrow).
