History/Physical:

67-year-old right-handed male.

Mild gait ataxia for two months.

Left sided dysmetria.

There is intrinsic T1 high signal in the left cerebellum, compatible with focal hemorrhage (*).

There is prominent rim enhancement.

The low signal rim is from hemosiderin (arrowheads). There is moderate vasogenic edema and mass effect (arrow).

Tx: Suboccipital craniectomy and resection of lesion.

Diagnosis: Metastatic adenocarcinoma (thyroid primary)

Epithelial tumour with a glandular, partly papillary architecture.

Papillary architecture with individual cells having mildly prominent nucleoli.

Abundant cytokeratin expression (brown).

Abundant thyroglobulin expression (brown).
