History/Physical:

87-year-old right-handed female.

Low thoracic back pain for two months.

The mass is low signal on T1 (*), replacing the normal fatty marrow which has high signal (arrow).

The mass is expansile (*). There is mild mass effect on the thecal sac (arrowhead). The margin with the vertebral body is well-defined (arrow).

In addition to the dominant T9 lesion (*), there are small lesions at T7, in the L1 spinous process (arrowhead) and the L3 vertebral body (arrow).

Diagnosis: Plasmacell tumour (multiple myeloma)

Discohesive cells with monocyte and plasmacytic morphologies including "clock-face" nuclei (arrow).
