History/Physical:

20-year-old right-handed female.

Mild gait ataxia for six months.

Progressive mild headache.

Complex solid/cystic mass in the 4th ventricle (*). Mass effect is minor. The prepontine cistern is not compressed (arrow).

There is distortion of the medulla (arrow).

The solid components enhance (asterisk).

The enhancement contacts the right posterior inferior cerebellar artery (arrow) and extends into the right foramen of Luschka.

Diagnosis: Ependymoma

Diffuse fibrillary lesion with tendency for perivascular rosettes (*) and curved nuclear arrays (arrow).

Ependymal rosette formation (*) by cells with round to oval nuclei.
