History/Physical:

28-year-old right-handed female.

Transient word finding difficulties.

There is a small T2-hyperintense lesion in the temporal pole subcortical white matter. There is subtle thickening of adjacent cortex (arrows).

The lesion is hypointense on FLAIR (*).

The lesion has low T1 signal (*), with a small focus of intrinsic high signal (arrow) which could be from mineralization.

Diagnosis: Ganglioglioma (with overlying cortical dysplasia – not shown)

Occasional neurons are binucleate (arrow).

Expression of synaptophysin among neuronal elements (arrow).
