History/Physical:

39-year-old right-handed male.

Transient episodes of experiencing unusual smells.

Mild headache for six months.

Nonenhancing mass in the medial right temporal pole (*). The middle cerebral artery is in the adjacent sylvian cistern (arrow).

The mass extends into the insula (*).

The mass is more conspicuous on the T2 sequence (*).

On T2, extension into the lentiform nucleus (arrowheads) and medial thalamus (arrow) can be appreciated.

Diagnosis: Oligodendroglioma, grade 2
