History/Physical:

75-year-old right-handed female.

Bitemporal hemianopsia for six months.

Large well-defined mass arising from the pituitary fossa (*). This has mass effect on the optic chiasm (arrows).

There is no invasion of the cavernous sinuses (arrows). This mass has sellar and suprasellar components with a waist at the diaphragma sella. This appearance is known as the 'snowman' sign.
