History/Physical:

56-year-old right-handed male.

Gradual hearing loss in left ear.

There is an ovoid mass in the left cerebello-pontine angle (*). This extra-axial lesion has mild mass effect on the left middle cerebellar peduncle (arrow).

This high resolution sequence shows the 'ice cream cone' sign, with the mass extending into the internal auditory canal (arrow).

Diagnosis: Schwannoma

Biphasic tumour of compact (Antoni A) streams of spindled cells alternating with rarefied (Antoni B) zones.

Nuclear palisades are present in compact (Antoni A) areas (arrows).
