History/Physical:

29-year-old left-handed male.

Multiple absence seizures difficult to control.

The cortex of the superior medial left fronto-parietal lobe is slightly thicker and more nodular than the right (arrows).

There is focal cortical thickening (arrow).

Diagnosis: Cortical dysplasia, type IIb

Normal neocortical lamination is replaced by a haphazard arrangement of randomly oriented dysmorphic neurons (note pial surface *).

Cytomegalic and ‘balloon’ cells are among the dysmorphic neuronal elements (arrow).
