History/Physical:

50-year-old right-handed male.

Low back pain and progressive leg weakness over one month.

Increased reflexes in lower extremities.

There is a pathological fracture with vertebral body retropulsion (*).

There is a prominent epidural soft tissue component (arrows). There is severe central canal narrowing and compression of the conus medullaris.

Most of the normal bone trabeculae are replaced by a lytic mass (*). The sclerotic bands in the mid-vertebral body may represent the margins of the mass.

Diagnosis: Giant cell tumour of bone

A spicule of bone (*) is surrounded by a pleomorphic cell population highlighted by scattered large multinucleated cells (giant cells).
