History/Physical:

44-year-old right-handed male.

Progressive headache over the past year.

The mass with irregular margins causes obstruction of the right lateral ventricle.

The mass has high signal on diffusion. This suggests hypercellularity.

The mass has solid and cystic components. The inferior margin of the mass blocks the foramen of Monroe, resulting in hydrocephalus.

Tx: Craniotomy and resection of brain tumour.

Diagnosis: Central neurocytoma

Monotonous population of rounded cells in enfilades and rosette-like arrangements.

Delicate branching capillaries, rounded tumour cell nuclei with perinuclear clearing (imitating oligodendroglioma).

Synaptophysin expression confirms neuronal lineage.
