History/Physical:

43-year-old left-handed male.

Generalized tonic-clonic seizure.

Subacute methemoglobin has intrinsic high T1 signal (*). There are abnormal flow voids anterior to the hematoma (arrows).

An enlarged draining vein enhances (arrow).

The lesion (*) is supplied by branches of the anterior cerebral artery (arrowheads). Drainage was predominantly into the superior sagittal sinus (arrow).

Drainage is primarily into the superior sagittal sinus (arrows) with some flow into the cavernous sinus (*).

Diagnosis: Arteriovenous malformation with haemorrhage

Large collection of pleomorphic vessels with intervening parenchyma (arrow) and hemosiderin deposits (*).

Similar features highlighted by Movat's pentachrome stain.

Thick-walled "arterialized" veins are abundant (*).
