History/Physical:

44-year-old right-handed female.

Three year history of complex partial seizures and progressive left hemiparesis.

Mild signal change in the lateral right frontal lobe (*). Subtle right cerebral atrophy. The right sulci are larger than the left (arrows).

Right Sylvian fissure (arrow) and temporal horn (arrowhead) are larger than the left. This is more evidence for right cerebral atrophy.

Foci of signal change in the right frontal operculum (arrow) and temporal operculum (arrowhead).

Diagnosis: Encephalitis, consistent with clinical diagnosis of Rasmussen encephalitis

Subtle inflammation in the form of lymphocytes and microglial aggregates (arrow).
