History/Physical:

31-year-old left-handed female.

Bilateral leg weakness, progressive over the past three months.

Increased lower extremity reflexes.

The normal thoracic cord is ovoid in the axial plane. Here the spinal cord is abnormally round.

There is a small cyst in the cord at C6/7 (arrow). The mass is difficult to perceive on this unenhanced image (*).

The well-defined intra-axial mass enhances avidly (*). There are rostral and caudal cysts (arrows).

The solid component is intermediate signal on T2 (*) with hyperintense rostral and caudal cysts (arrows).

Diagnosis: Spinal ependymoma

Fibrillary tumour with perivascular clearing (arrow).

Nuclear monotony and perivascular clearing and some rosette formation (arrow).
