History/Physical:

46-year-old right-handed female.

Progressive headache over one year.

There is a large bilobed cystic lesion in the left frontal lobe. The slight hyperintensity in the cyst (*) may be from CSF pulsation rather than proteinaceous debris.

The cyst may be intra-axial (arrow). There is no cortical grey matter at the margin, and no CSF cleft.

The midline shift is apparent on this coronal slice (arrow). There is more space beneath the falx , allowing for more herniation. On the prior coronal image, which is more anterior, the falx covers more of the midline and limits the extent of subfalcine shift.
