History/Physical:

59-year-old left-handed female.

Three year history of lethargy, polydipsia, and hypersomnia.

There is a mass within the 3rd ventricle (*).

There is obstructive hydrocephalus, with enlarged lateral ventricle atria bilaterally (arrows).

The small low signal foci could be from calcification (arrows).

Diagnosis: Craniopharyngioma

Fibrovascular elements and pools of wet keratin (*) are bordered by a basaloid layer (arrow) and squamous elements (arrowhead).
