History/Physical:

39-year-old left-handed female.

Headache over the past year. Parinaud’s syndrome.

The pineal region mass has mass effect on the superior colliculi (arrowhead) and aqueduct (arrow). There is spread along the floor of the 3rd ventricle (*).

The well-defined mass enhances (*).

There is obstructive hydrocephalus (arrowheads).

Diagnosis: Germinoma

Hypercellular, biphasic tumour with large epithelioid cells and lymphocytic infiltrates.

Higher magnification of the same demonstrating epithelioid tumour cells (arrow) and intervening lymphocytes (arrowhead).
