47-year-old left-handed female.

Recent onset of repetitive movements such as picking at clothes.

Movements preceded by déjà vu memories.

The right hippocampus is smaller than the left (arrowheads). The right temporal horn is larger the left (arrows).

The right hippocampus has increased signal compared to the left (arrows).

Diagnosis: Mesial temporal sclerosis

Gliosis within the end-plate (CA4) region (*).
