History/Physical:

45-year-old right-handed female.

Headache for three months.

Large mass in the right lateral ventricle (*).

The mass enhances avidly.

High signal on diffusion suggests this mass is hypercellular.

The mass arises from the septum pellucidum, a midline structure which separates the lateral ventricles.

Diagnosis: Central neurocytoma

Monotonous tumour cells with round, finely stippled nuclei are arranged in rosettes (*) and curved linear runs on a fibrillary background.

Microtubule associated protein expression is abundant.

Neuronal nuclear antigen (NeuN) expression.

Synaptophysin expression.
