History/Physical:

86-year-old right-handed male.

Moderate headache for one year.

Recent onset of mild right-sided weakness.

Previous left frontal craniotomy (arrows). Significant recurrent tumour (*).

There is heterogeneous enhancement.

Tx: Craniotomy and removal of tumour.

Diagnosis: Meningioma, clear cell

Relatively patternless tumour cells show vague lobulation and streaming.

Cytoplasmic clearing is apparent on higher magnification.

Many lesional cells express epithelial membrane antigen.

Widespread vimentin expression.
